' "Shouldering his pack and setting forth, he crossed the river ... " No, he didn't. Not unless his pack was in the river' Yes he did, the comma implies a clause and the action is therefore separate. The problem with this kind of prescriptive approach to creative writing is that its useless if you dont need it, and not much help if you do.
