I use PmWiki for several projects with the organization I work for. It is well documented, easy to set up, and simple to build templates (skins) for it.

PmWiki is a open source PHP based wiki. Extremely simple to setup and even easier to maintain - this is one of the best Wikis available. All it needs is a web server (lighttpd is suggested by me) and nothing else. It can also be run stand alone that is locally without a web server. The wiki stores its pages in flat files instead of a DB. It scales up nicely and has plugins/cookbook recipes for almost anything you would like to do on it. Being very modular its very simple and easy to add new features to PmWiki on your own too.

My favorite wiki for a hosted environment. Doesn't require a database, is lightening quick, and has tons of really cool plugins.

Along with dokuwiki and mediawiki this is a nice product and easy to set up.
