The characterization of criminal negligence, professional misconduct, battlefield incompetence, concealment and destruction of evidence, deliberate deception, and conspiracy to deceive are not "missteps."
This isn't actually new (I gave a presentation last year that included some of this information). No one really knows the true frequency of this type of twinning. Along with mosaicism and chimerism, these genetic eccentricities can be quite difficult to detect without DNA analysis. No one was looking for this type of twin so they were often mis-characterized as "fraternal" (di-zygotic) twins.
